                  data should be treated as arising from different individuals
                  (this occurred for subject number 23).

  2               Elapsed time (hours)

  3               Quinidine concentration (mg/l)

  4               Dose (mg of quinidine base)

  5               Dosage form (ignore).  0=quinidine sulfate  1=quinidine
                  gluconate

  6               0=observation, otherwise 1

  7               0=observation, 1=dose event, otherwise 2 (change in one of the
                  other variables)

  8               1=steady state dosing, 2=overlay a second dosing regimen on
                  top of the first (this happened once for subject 50),
                  otherwise 0 (the original analysis used this to calculate the
                  contribution of a steady-state dosing regimen using item 9 as
                  the interdose interval.)

  9               >0=interdose interval for steady-stage dosing, otherwise 0

 10               Age (years)

 11               Height (inches)

 12               Weight (kg)

 13               Race/ethnicity: 0=Caucasian, 1=Latin, 2=Black

 14               Smoking status: 1=smoker, otherwise 0

 15               Ethanol abuse: 0=no ethanol consumption or social drinker,
                  1=current ethanol abuse, 2=no current ethanol abuse but
                  history of abuse

 16               Congestive heart failure: 0=no or mild, 1=moderate, 2=severe

 17               Creatinine clearance: 0,1,2,4,7=creatinine clearance > 50
                  ml/min, 3,5,6,8=creatinine clearance < 50 ml/min
                  1=chronic liver disease, 7,8=acute liver disease,
                  5,6=hemoperfusion or dialysis, 4=not used 

 18               albumin concentration (g/dl) (ignore)

 19               alpha-1-acid glycoprotein concentration (mg/dl) (this measures
                  the level of a circulating molecule that binds quinidine.)
